appointmentsAt our office, we know that your time is important. If you are a new patient, please arrive at our office 20 minutes prior to your scheduled time.
If you are a returning patient we ask that you arrive 10 minutes before your scheduled appointment. This extra time allows us to get you through the check in process and update any information that is needed. Appointments are an important part of your treatment, as time has been reserved specifically for you to talk with the doctor and receive treatment. Please arrive on time for your appointment. If you are more than 15 minutes late, you might be asked to reschedule your appointment. We realize unexpected circumstances arise when you cannot keep your reserved appointment. If this occurs, we ask that you give us a call 2 days in advance to cancel or reschedule your appointment. This will allow us to give the appointment time to another patient. We do reserve the right to charge a fee of $25.00 for a broken appointment. We encourage patients to schedule appointments in advance. |

Our doctors are participating providers with most major dental insurance plans. Please have your correct dental insurance information available when calling the office to schedule your appointment so we can confirm that we are a participating provider with your dental insurance plan.
Dental Insurance is an agreement between a patient and an insurance company. Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be. The insurance carrier agrees to pay certain prescribed benefits at a predetermined maximum rate. Typically, there are limitations to dental policies. That is, there are deductibles; co-payments and some procedures may not be covered at all. Each policy is different and we probably don't know the fine details of your policy. We encourage you to speak directly to your insurance carrier or employer regarding what services your policy covers. The fees charged for services rendered to those who are insured, are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months. We will estimate to the best of our ability the amount you are responsible for paying for the service. These are not guarantees that the insurance company will pay the amount left. You are responsible for paying your portion for services at the time the service is rendered. Your portion would include deductibles and any non-covered procedures. For your convenience, we accept Visa, MasterCard, cash, checks and debit cards. We will gladly bill your insurance carrier as a courtesy to you. Once the insurance payment has been applied, any remaining balance on the account is ultimately your responsibility and is expected after you receive your statement. We appreciate your cooperation in adhering to our financial policies and assure you we will continue to do our best to provide your family with optimal dental care. payment options
For our patients without insurance, payment is expected at the time services are performed. For your convenience, we accept cash, personal checks, money orders and major credit cards. When more extensive dental care is necessary, financial arrangements can be made with our office.
We also offer 6-month or 12-month, interest-free monthly payment plans through CareCredit to make dental treatments affordable to our patients. Care Credit is a card used exclusively for dental or medical services. We have the applications or you may go online to apply. There is a $30.00 fee charged on all returned checks. Because of increasing cost of postage, we try to send as few statements as possible. When you get a statement, it means that the balance is what you owe, as insurance has either paid or denied the claim. Any balance not paid in 90 days from the first statement date will be turned over to the Collection Bureau and assessed a charge of 33% of the balance. It is understood and agreed that if an account is placed for collection you waive your rights under HIPPA to release the following information only to the collection agent: your name, address, home, phone, work phone, Social Security number. No medical or dental information will be released to a non-practitioner. |
